Do you accept insurance for your services?
No, Evergreen Sports Medicine does not bill insurance for services. We are a cashed based sports medicine business that provides personilized treatment and rehab without the hassle of insurance. I accept cash and all major credit cards.
What happens if i need to cancel or reschedule my appointment?
All cancelations must occur with a minimum of 24 hours notice. All notices less than 24 hours before scheduled appointment will result in a non-refundable, non-transferable fee. No call, no shows will result in a full service cost to the card on file.
